Our fire place heat goes straight up! We put a ceiling fan up there to blow all the heat back down, but the bugger is unbalanced and starts to swing dangerously on medium or high! We actually need to hire someone to come in and balance it, because there is no easy way to get to it, other than scafolding.
I put one in a similar cathedral ceiling. We went from 12 cords of wood to 10 and the house was noticeably warmer.
