Looks like a common GM midsize for the '70-80s, used by the majority of aftermarket conversion kit manufacturers. Below is a pic of one I've got in a box somewhere, though pictured from the bottom you should see the similarity. I'm in the early process of moving into the new garage so don't know where it is to get a part number for you......if I find it before anyone else gives you a lead I'll post up.
As for bending back the noise maker that should be considered a "get bye" cludge. The pad is likely glazed enough that it may not be fully effective anyway.Your Uncle Bob, Senior Geezer Curmudgeon
It's much easier to promise someone a "free" ride on the wagon than to urge them to pull it.
